Some textures are kind of rubbish but the most part of world design is just wonderful. The rooms, hallways, warehouses, and every piece of confined space is built to create an atmosphere that makes you feel uneasy. Paranoia 2: Savior aims to be a horror game and for that the audio effects are also very well chosen.
